Medical rehabilitation and frailty
We are one of the leading trusts in the UK, and our Medical Rehabilitation and Frailty service assesses and treats a wide range of clinical presentations across the general medicine and frailty specialities. The team ensures the highest quality of therapy on the wards for patients under our care, and is made up of physiotherapists, occupational therapists and therapy assistants who work with individuals to help them recover after a period of ill health.
We are focussed on ensuring that any hospital stay is minimised, and have strong links with community partners across the local boroughs who facilitate on-going rehabilitation as part of our discharge to assess service. Possessing a strong evidence based clinical knowledge set, we work closely with colleagues from other specialities where needed to ensure that health outcomes are maximised, and always strive to offer a truly patient centred service that holistically meets the needs of all under our care.
Our team
The team consists of occupational therapists and physiotherapists who are all registered members of the health and care professionals’ council, and is overseen by a Clinical Lead therapist who is responsible for the overall quality of the service.
Within the team, we also have therapy assistants who provide valuable support on the wards, and ensure that rehabilitation plans are carried out.
