Request your health records
To request a copy of your own (or a dependent’s) health records it would be helpful if you could complete the Access to Health Records Subject Access Request (SAR) application form, below:

Requests can be verbal or written with or without the form but in all cases the request will need to be recorded accurately and ID obtained to prove identity. The form saves time and confusion for both the requester and the Trust.
Care Information Exchange (powered by Patients Know Best)
The Care Information Exchange is an online personal health record portal for patients and clinicians. It is provided and supported by the company ‘Patients Know Best’ and is in use across much of North West London and the UK, including Imperial College Healthcare NHS Trust and ourselves.
The goal of the CIE is to help you feel more in control of your health through online access to your personal health record. We hope to improve your hospital visits by making sure you and your team of health professionals have access to the most up-to-date Information. Currently, more than 50,000 patients across 11 Hospitals in North West London have registered for the CIE.
With the CIE you can:
- view your hospital letters, appointment details, test results and scans
- record important medical information about yourself, such as allergies and diagnoses
- track and monitor your health and symptoms, such as blood pressure and weight
- access a library of health resources and care plans to help you manage your condition
- share your records with family, carers and health professionals

You can access and use CIE on any home computer, smartphone or tablet and you don’t need any specific software to get started. If you don’t use the internet, you can have a family member or carer manage your patient record for you. All you need to do is provide their email address along or instead of yours and the team will create a record for you both.
CIE is secure and easy to use—it has been designed specifically for use by patients and clinicians. All your data is encrypted in both transit and storage.
